Preparing for Your First Visit at Eye Care Specialists

We look forward to welcoming you and your family to Eye Care Specialists. Your comprehensive eye examination and required testing may take two hours, depending on your appointment type.

Your exam may require that your eyes be dilated. For your comfort and safety, we recommend you bring someone with you to drive you home after your visit. A full report of all findings will be sent to your referring doctor following your examination.

Please bring the following information with you to every appointment:

  • A complete list of current medications, including your dosage and how often you take each medication.
  • All current eyewear including your eyeglasses, contact lenses and your latest eyewear prescription.
  • All insurance plan cards including Medicare and private pay coverage.
  • A referral form from your primary care physician if required by your insurance carrier.
